Document glossary for applicants

To evaluate your admission eligibility in a timely manner, we need you to submit all documentation as outlined in your UOIT Admissions account. Below is a comprehensive glossary of various admission documents that may be required by an applicant. Please only submit what is outlined specifically in your UOIT Admissions account.

Activities prior to application Written letter detailing your activities (professional and academic) since you were last in secondary school
Certified translation transcript Certified translation of transcript
Copy of interim transcript Unofficial interim (in progress or midterm) transcript
Copy of nursing license Proof of College of Nurses of Ontario (CNO) licence number from the CNO Find a Nurse website (printout) or copy of CNO certificate
Copy of transcript Unofficial final transcript
Course descriptions Course descriptions for transfer credit evaluation
Course outlines Post-secondary course outlines for transfer credit evaluation
Course syllabus Course syllabus for transfer credit evaluation - a complete overview of the entire course, including learning objectives
Credential assessment Credential assessment - International Credential Assessment Service of Canada or World Education Services
Deferral request Request to defer your application to a future academic term (maximum one calendar year)
English proficiency letter Proof of English language proficiency from an academic institution
English proficiency not met English proficiency examination has not met the requirements
Extenuating circumstances letter A letter outlining extenuating circumstances
Final high school transcript Official high school (secondary school) transcript
Final transcript Official final transcript; cannot have any in progress or pre-enrolled courses listed
IB/AP transcript International Baccalaureate (IB) or Advanced Placement (AP) transcript
Interim high-school transcript Official interim (in progress) transcript
Interim transcript Official interim (in progress) transcript
Letter of intent Letter of intent or personal statement

Miscellaneous

Documents that are not required for admission consideration. These should not be submitted; however, if they are, they will be labelled as such.
Notarized copy of transcript Notarized copy of transcript (in cases where extreme circumstances exist such as refugee claimants, applicants from countries in turmoil, etc.)
Passport Copy of passport - this can be a scan or image of the main page of the document
Permanent residency card Copy of permanent residency card
Personal profile Supplementary application for the Bachelor of Education program
Predicted grade (IB/AP) Unofficial predicted IB or AP transcript
Proof of credential conferred Official final college or university transcript, stating academic credential has been awarded
Proof of degree conferred Official final college or university transcript, stating academic credential has been awarded
Proof of English language proficiency Proof of English language proficiency; see English language proficiency page for more details
Proof of enrolment (upgrading) Proof of enrolment for upgrading course(s) - must include course code, start and end dates, and insitution where upgrading is taking place
Proof of health insurance Not required for admissions purposes; contact UOIT International for further information
Proof of OSSD Proof of receiving an Ontario Secondary School Diploma (OSSD), final high-school transcript
Release of information authorization Information authorization disclosure form
Resumé Current resumé or curriculum vitae
SAT test score Official Scholastic Aptitude Test (SAT) scores from CollegeBoard.org
Upgrading interim mark Interim (in progress) upgrading transcript
Upgrading mark Official transcript of upgrading course(s)
WAEC results Official West African Examinations Council (WAEC) scratch card
